Within 30 days of the birth, you must:

1. complete and sign the “Self-declaration of birth" form;
2. provide the following information in the form:
  - your personal details and Italian tax code;
  - your Specialist School and year of training;
  - the child’s first name and surname;
  - the child’s place and date of birth;
  - the child’s Italian tax code;
3. send the completed form from your institutional `@unibs.it` email address or personal certified email address to [email protected].
